Job summary

Brookshire Post Acute in Denver, CO is hiring a Physical Therapist to join our compassionate team. You will provide physical therapy services to residents and work closely with nurses and therapists to achieve positive outcomes.

A CO license and post-acute care experience are preferred. NetHealth and PCC proficiency are a plus, and a $3,000 bonus is available for full-time employees who remain in status through milestones.

Qualifications

• Current, unencumbered Physical Therapist license in CO.
• Experience in a post-acute or skilled nursing facility a plus.
• Proficient with NetHealth and Point Click Care is a plus.

Responsibilities

• Provide physical therapy services to residents.
• Collaborate with care team to coordinate patient rehabilitation.
• Maintain accurate documentation and progress notes in the EHR.
