Responsibilities

• Perform initial and ongoing systematic patient assessment.
• Promote continuous quality improvement.
• Teach and counsel patients/families.

• Work cooperatively to identify and solve patient‑specific and facility‑wide needs.
• Participate in discharge planning for each patient, including placement, patient/family education, and adaptive equipment.
• Supervise Physical Therapy Assistants and supervise PT and PTA students.
• Conduct individual patient therapy regimens.
• Monitor patients' response to treatment and modify treatment during sessions as needed.
• Complete appropriate documentation according to department policies and procedures.
• Participate in departmental, hospital, and community continuing education seminars and in‑services.

Minimum Qualifications

• State licensure: Hold a current state licensure as a Physical Therapist (PT).
• Certified BLS: Possess Certified BLS or obtain it within 90 days of employment.
