Stern at Home Therapy is seeking a licensed Physical Therapist (PT) to provide one-on-one home health visits in and around Avenel, NJ. This PRN or part-time W-2 opportunity offers flexible scheduling and a clinician-driven caseload with autonomy over evaluations and treatment plans.

You will perform evaluations, design individualized care plans, and help geriatric patients improve mobility, balance and safety in their homes, with support from an established clinical and administrative team.

Qualifications

• An active Physical Therapist (PT) license in New Jersey is required.
• Reliable transportation required.
• Strong clinical, communication, and time-management skills.
• Ability to work independently in a home-based setting.
• Home health experience is preferred but not required.

Responsibilities

Physical Therapist (PT) – Home Health | Avenel, NJ | PRN & Part-Time | $65–$85 Per Visit

Stern at Home Therapy is hiring a Physical Therapist (PT) to provide one-on-one care to patients in their homes in and around Avenel, NJ.

Choose between part-time W-2 employment with benefits or a flexible PRN opportunity based on your availability.

This opportunity is ideal for a licensed Physical Therapist looking to build a steady part-time caseload or add flexible PRN visits to an existing schedule without the double-booking, productivity quotas, and clinic burnout common in traditional outpatient settings.

Treat one patient at a time, manage your availability, and provide individualized care with support from an established clinical and administrative team.

What You'll Do:

As a Physical Therapist with Stern at Home Therapy, you will evaluate and treat a primarily geriatric patient population in their homes throughout the Avenel area.

Responsibilities include:

• Perform physical therapy evaluations and develop individualized plans of care.
• Provide one-on-one treatments based on each patient's functional needs and goals.
• Manage individualized treatment plans with clinical autonomy.
• Help patients improve mobility, strength, balance, safety, and overall function.
• Complete, accurate, and timely clinical documentation.
• Communicate with patients, families, physicians, and members of the clinical team.
• Coordinate with our administrative team regarding scheduling, patient care, and logistics.

Compensation:

Part-Time W-2:

• $65 per follow-up visit.
• $75 per 4-unit evaluation.

PRN:

• $72 per 4-unit follow-up visit.
• $85 per evaluation.
