Job summary

Therapy Management Corporation (TMC) is seeking a licensed Physical Therapist to empower adult and geriatric patients by improving mobility, independence, and quality of life through patient-centered care.

You will work in a collaborative clinical environment, follow patient progress over time, and focus on meaningful outcomes while maintaining clinical autonomy and professional growth within a supportive team.

Qualifications

• Licensed PT in the practicing state.
• Graduate of an accredited Physical Therapist Program.
• Interest or experience in skilled nursing, long‑term care or geriatric rehabilitation.
• Passion for patient-centered care and improving quality of life for older adults.

Responsibilities

• Evaluate patients and deliver personalized PT treatments to improve mobility and independence.
• Develop and implement patient-centered treatment plans with long-term goals.
• Provide hands‑on interventions to support recovery and safety in daily activities.
• Collaborate with interdisciplinary teams and caregivers to ensure continuity of care.
• Track progress and adjust plans based on rehabilitation outcomes.
• Maintain accurate clinical documentation and ensure licensure compliance.
