Job summary

Trinity Healthcare Talent Services is seeking a skilled Physical Therapist to join our team at a 74-bed Skilled Nursing Facility. You will evaluate patients, develop individualized treatment plans, and help them regain mobility and improve quality of life.

This position offers 35 hours per week (Mon–Fri, with weekends as needed) and compensation of $43–$46 per hour. Candidates must hold a current state license, have CPR certification, and at least two years of SNF experience.

Qualifications

• Bachelor's, Master's, or Doctorate in Physical Therapy from an accredited program.
• Current state license to practice as a Physical Therapist.
• CPR certification.
• Minimum of 2 years of experience in a skilled nursing facility or similar setting.
• Excellent communication and interpersonal skills.
• Strong time-management and organizational abilities.
• Ability to work independently and as part of a team.

Responsibilities

• Conduct thorough evaluations of patients' physical abilities and limitations.
• Develop and implement personalized treatment plans to address patients' specific needs and goals.
• Utilize a variety of techniques and modalities to help patients improve their strength, balance, flexibility, and overall physical function.
• Educate patients and their families on proper exercises and techniques to continue their progress outside of therapy sessions.
• Monitor and document patients' progress and adjust treatment plans as needed.
• Collaborate with other healthcare professionals, such as doctors, nurses, and occupational therapists, to ensure comprehensive care for patients.
• Maintain accurate and up-to-date patient records.
• Stay current with advancements and best practices in physical therapy through continuing education and professional development opportunities.
