Granite Peak Home Health in Montana is seeking a licensed Physical Therapist to join our growing home health team, delivering patient-centered care in patients' homes and applying local market knowledge to outreach activities.

Qualifications:
Graduate from a physical therapist curriculum approved educational institution
Currently licensed in the state of Montana
1 year home health experience, preferred
BLS/CPR required for this position
Knowledgeable with healthcare regulations, HIPAA compliance, and patient privacy requirements
Knowledge of the local healthcare market and community outreach strategies
Knowledge of appropriate software including Microsoft Word, Email, Outlook, and Wellsky (EHR)
Clean driver’s license with a clean driving record
Principal Responsibilities:
Understands and adheres to established policies and procedures
Provides physician-prescribed physical therapy
Improves or minimizes residual physical disabilities of the patient
Returns the individual to optimum and productive level within the patient's capabilities
Participates with all other home care personnel in patient care planning
Directs and supervises personnel as required
Responsible for initial assessment, plan of care, maintenance program development, and modifications and reassessments every 30 days
Performs all skilled procedures as ordered by the physician
Consult with physicians regarding changes in treatment
Writes reports to physicians regarding patient's progress
Instruct patients and family/significant others in-home programs and activities of daily living
Participates in in-service programs and presents in-service programs as assigned
Participates in Quality Assurance and Performance Improvement activities as assigned
Attends all patient care conferences as scheduled
Prepares clinical and progress notes
Assists physician with evaluating level of function
Helps develop the plan of care and revise it as necessary
Consult with family and Agency personnel
Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant changes in condition under Agency defined time frames
