Napa Valley Care Center in Napa, CA is seeking a Physical Therapist to join our rehabilitation team to deliver high-quality post-acute care and improve residents’ independence.

We offer starting pay at $70/hour (DOE) with Full-Time, Part-Time, or PRN opportunities. The role involves developing individualized treatment plans and collaborating with an interdisciplinary team to maximize resident outcomes.

Qualifications

• Current and active California Physical Therapist (PT) License.
• Strong communication and interpersonal skills.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide skilled physical therapy services to improve mobility, strength, balance, endurance, and functional independence.
• Implement evidence-based rehabilitation interventions.
• Collaborate with physicians, nurses, occupational therapists, speech therapists, and interdisciplinary care teams.
• Educate residents, families, and caregivers on therapy goals, treatment plans, and safety techniques.
• Document patient progress accurately and timely in accordance with regulatory and facility standards.
• Assist residents in achieving their highest practical level of function and quality of life.
