Community Health at Home is seeking a Physical Therapist to deliver in-home rehabilitation services across North and Central Marion County. You will assess mobility, design personalized treatment plans, and coordinate with families and other clinicians to promote recovery and independence.

This role offers a competitive salary, a $10,000 sign-on bonus, and generous mileage reimbursement of 58.5 cents per mile; typical shifts are 8 hours, Monday through Friday, with emphasis on patient-centered

Formación

• Bachelor's degree in Physical Therapy from an American PT program.
• Active PT license in the state you practice.
• CPR/BLS certification current and compliant with regulations.
• One-year physical therapy experience is preferred but not required.
• Current driver's license and ability to travel for home visits.

Responsabilidades

• Conduct evaluations of patients' physical abilities, mobility, strength and functional limitations.
• Implement therapeutic exercises, manual therapy and modalities to improve function.
• Educate patients and families on exercises, safety and home-care modifications.
• Monitor progress and adjust treatment plans to achieve optimal outcomes.
• Maintain accurate patient records per state regulations and agency policies.

8-hour Day Shift Mon - Fri (8 AM-5PM)

We are seeking a skilled and compassionate Physical Therapist to join Community Health at Home team. The successful candidate will provide physical therapy services to patients in their homes, promoting rehabilitation, recovery, and improved quality of life. As a key member of our interdisciplinary team, you will work closely with patients, families, caregivers, and other healthcare professionals to develop and implement personalized treatment plans that address each patient's unique needs and goals.

Responsibilities

• Conduct evaluations of patients' physical abilities, mobility, strength, and functional limitations.
• Implement therapeutic exercises, manual therapy techniques, and modalities to improve patients' physical function and mobility.
• Educate patients and their families about exercises, safety, and lifestyle modifications to promote recovery and prevent re-injury.
• Monitor and document patient progress, adjusting treatment plans as necessary to achieve optimal outcomes.
• Maintain accurate and up-to-date patient records in compliance with state regulations and agency policies.

Job Requirements

• Bachelor's degree from an accredited school approved by the American Physical Therapy Association
• Active registered PT license in the state which you practice
• Must obtain and maintain current CPR/BLS Certification
• One-year physical therapy experience in teaching or clinical setting is preferred, but not required.
• Current Drivers License
• Comply with federal, state and local laws and regulations
