Responsibilities
- Provide evidence-based physical therapy evaluations and treatment plans.
- Monitor patient progress and adjust interventions to meet treatment goals.
- Meet or exceed quarterly performance targets and contribute to pipeline planning.
- Document sessions and outcomes clearly in the patient records system.
- Collaborate with physicians and care team to optimize therapy plans.
- Deliver clear, measurable results within defined timeframes to maintain high visibility of work.
- Adhere to all safety, compliance, and facility policies and procedures.
- Identify opportunities for process improvements to increase efficiency and outcomes.
- Demonstrate professional growth through continuing education and skill development.
- Educate patients and families on home exercise programs and self‑management.
- Actively participate in team meetings to share updates and align on goals.
Experience/Qualifications
- Licensed physical therapist with valid state license and CPR certification.
- Experience in evidence-based PT evaluation and individualized treatment plan development.
- Ability to monitor progress and adjust interventions to hit targets.
- Accurate documentation of sessions and outcomes in EMR system.
- Adherence to safety, compliance, and facility policies at all times.
- Interdisciplinary collaboration with physicians and care team.
- Deliver measurable results within defined timeframes and maintain high visibility.
- Pursuit of continuing education and skills development for professional growth.
- Educate patients and families on home exercise programs and self‑management.
- Identify and implement process improvements to improve efficiency and outcomes.
- Active participation in team meetings to align goals and share updates.
- Contract role: comfortable with contract-based engagement and performance-based rewards.
- On-site work preference compatible with the role's location requirements.
