Job summary

Caring Hand Home Health Solutions, Inc. in California is seeking a Physical Therapist to deliver skilled services in patients' homes, helping them regain mobility and independence.

The role focuses on evaluating function, developing plans of care, and collaborating with physicians and the care team. This per diem position offers flexible scheduling, a competitive per-visit rate of $160–$180, and opportunities for full-time conversion based on performance.

Qualifications

• Bachelor’s or Master’s degree in Physical Therapy from an accredited program.
• Active California Physical Therapy license.
• Minimum 2 years of PT experience; home health background is a plus.
• Current CPR certification (AHA).
• Valid driver's license and reliable, insured vehicle.

Responsibilities

• Evaluate patients' functional status, mobility, muscle strength, gait, and ROM to develop individualized treatment plans.
• Deliver physical therapy services per physician's plan of care and California's Physical Therapy Practice Act.
• Educate patients and families on exercises, equipment use, and recovery goals.
• Collaborate with physicians and the care team—reporting changes in patient condition and participating in case conferences.
• Supervise Physical Therapy Assistants and oversee Home Health Aide care bi-weekly when PT is the sole skilled service.
• Manage documentation including clinical progress summaries and discharge planning.
• Identify needs for additional home health services and coordinate referrals as appropriate.
• Perform other duties as required for patient services.
