St. Croix Health in Wisconsin is seeking a full-time Physical Therapist to join our compassionate team. This position will float between our Webster and Frederic Clinics, evaluating and treating patients of all ages based on evidence-informed practice.

The therapist delivers quality care, documents thoroughly, and collaborates with care teams and referral sources. Requirements include a PT degree, WI license, BLS, and 2+ years’ experience; specialist certification preferred.

Qualifications

• Graduate of approved physical therapy program.
• Specialist Board Certification preferred.
• WI Physical Therapist license required; MN license preferred.
• Current BLS Certification.

Responsibilities

• Evaluate and treat patients of all ages with varied impairments.
• Deliver evidence-informed physical therapy and measure outcomes.

• Maintain relationships with referral sources and patients.

St. Croix Health is seeking a full-time Physical Therapist to join our compassionate team of professionals. This position would float between our Webster and Frederic Clinics.

This position is responsible for the evaluation and treatment of patients of all ages presenting a broad range of impairments and functional limitations. The physical therapist is responsible for delivering evidence-informed treatments and objectively measuring the outcomes of treatment. This position provides quality patient care, assisting with in-servicing rehab staff, other medical center departments, and the community on wellness and rehabilitation topics.

Essential Duties and Responsibilities

Evaluate and provide physical therapy treatment to child, adolescent, adult, and geriatric patients based on evidence-informed best practice patterns in physical therapy. This includes performing appropriate documentation responsibilities.

• Stay current on physical therapy related practices, wellness and rehabilitation topics
• Deliver quality patient care
• Complete required charting & documentation responsibilities in a timely manner
• Maintain and foster relationships with referral sources and patients.

Direct, supervise, assess and communicate with Physical Therapist Assistants.

• Oversee work performed by Physical Therapist Assistants as outlined in the state statute
• Partner with Physical Therapy Assistants to ensure completeness of PT plan of care (POC)
• Review charting activities and provide feedback

Other duties as assigned.

• Participate in inter-departmental, committee-based, and community educational outreach on topics related to wellness, sports performance, and physical rehabilitation
• Assist Manager of Rehab Services with program development in specific areas of practice

Requirements

Education & Licensure

• Graduate of approved physical therapy school
• Specialist Board Certification preferred
• Valid/current WI Physical Therapist license required. MN Physical Therapist license preferred
• Current BLS Certification

Experience

• 2+ years of experience preferred
