Legacy Healthcare Services is seeking a skilled Physical Therapist to join our team. This salaried position is based on a 30-hour workweek and emphasizes compassionate, patient-centered care for senior residents in a collaborative setting.

The role involves planning and delivering individualized rehabilitation programs, coordinating with a multidisciplinary team, and participating in clinical conferences to refine the plan of care. Strong interpersonal and clinical skills are essential.

Responsibilities

• Provide rehabilitative services to patients under specific medical orders as part of a coordinated team.
• Plan initial and subsequent treatment programs based on clinical findings.
• Participate in clinical conferences to coordinate the plan of care for patients.
• Treat residents one on one with individualized private sessions using a functional-based approach.
• Customize treatment sessions to individual patient needs.

Pay Range

USD $80,000.00 - USD $82,500.00 /Yr.
