Timber Ridge Health & Rehab in Stevens Point, WI seeks a Full-Time Physical Therapist to join our in-house rehab program. You will screen and evaluate patients, develop and implement care plans, and provide rehabilitative treatments to support recovery in a post-acute setting.

With licensure required and new grads welcome, you will deliver wound care, manual therapy, therapeutic exercises, and patient counseling while maintaining regulatory compliance and contributing to a high-quality discharge

Qualifications

• Licensed Physical Therapist required.
• New grads welcome to apply.
• Active PT license in good standing is required.

Responsibilities

• Screen and evaluate patients with physical functioning disorders and wounds; develop care plans.
• Provide rehabilitative and medically necessary treatments per regulatory standards.
• Perform procedures in the rehab plan, including manual techniques, ambulation, therapeutic exercises, modalities and wound care.
• Offer counseling to patients, families, and other providers about physical disorders.
• Prepare comprehensive discharge summaries in compliance with regulations.

PHYSICAL THERAPIST - FULL TIME - TIMBER RIDGE HEALTH & REHAB - STEVENS POINT, WI

Timber Ridge Health & Rehab in Stevens Point, WI, is a post-acute care facility with long-term care, short-term care, and geriatric services provided. We can offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery. We are currently seeking a Full Time Physical Therapist for our in-house rehab program!

Key Responsibilities:

• Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans adhering to regulatory and clinical standards.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements.
• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.

Qualifications:

Physical Therapist license is required.
