Job summary

Azza HealthCare Agency seeks a licensed Physical Therapist to deliver patient-centered care in a hybrid acute/inpatient setting. You will evaluate patients, develop and implement treatment plans, and collaborate with care teams, families, and patients.

You will educate patients and families, document treatments accurately, delegate tasks wisely, and participate in interdisciplinary rounds to optimize outcomes while supporting a growth-focused team environment.

Qualifications

  • Bachelor's in PT is required; Master’s preferred.
  • Licensed PT with current credentialing is required.
  • Ability to design and adjust treatment plans based on discharge goals and outcomes.
  • Strong communication for patient and family education and team collaboration.
  • Experience with assessment, reassessment, and consultation in PT services.

Responsibilities

  • Provide skilled PT services including evaluation and treatment.
  • Educate patients, families, and healthcare providers.
  • Delegate tasks to assistants and students per skill level.
  • Complete accurate documentation per policies.
