Job summary

CommonSpirit Health at Home in Springfield, OH is seeking a skilled Physical Therapist to join the Community Mercy Home Care team. You will provide in-home PT services, developing personalized treatment plans to improve mobility and quality of life for patients in their homes.

The role emphasizes functional movement, collaboration with an interdisciplinary team, and patient/family education. A $10,000 sign-on bonus is offered to eligible applicants.

Qualifications

• Bachelor’s degree required from an American Physical Therapy Association approved program.
• Active licensed Physical Therapist in the practicing state.
• CPR/BLS certification required.
• One year PT experience preferred but not required.
• Valid driver’s license.

Responsibilities

• Conduct evaluations of patients’ physical abilities, mobility, strength, and functional limitations.
• Implement exercises, manual therapy, and modalities to improve function.
• Educate patients and families on exercises, safety and lifestyle modifications.
• Monitor progress and adjust treatment plans to achieve optimal outcomes.
• Maintain accurate patient records in compliance with regulations.

We are seeking a skilled and compassionate Physical Therapist to join Community Mercy Home Care team. The successful candidate will provide physical therapy services to patients in their homes, promoting rehabilitation, recovery, and improved quality of life. As a key member of our interdisciplinary team, you will work closely with patients, families, caregivers, and other healthcare professionals to develop and implement personalized treatment plans that address each patient's unique needs and goals.

The focus of Physical Therapy is to improve functional movement. Practicing in the home setting, where patients spend most of their time, is the most "functional" place to make an impact on this!

Responsibilities:

• Conduct evaluations of patients’ physical abilities, mobility, strength, and functional limitations.




• Implement therapeutic exercises, manual therapy techniques, and modalities to improve patients’ physical function and mobility.




• Educate patients and their families about exercises, safety, and lifestyle modifications to promote recovery and prevent re-injury.




• Monitor and document patient progress, adjusting treatment plans as necessary to achieve optimal outcomes.




• Maintain accurate and up-to-date patient records in compliance with state regulations and agency policies.

Qualifications:
