Boost Home Healthcare is seeking a licensed Physical Therapist to deliver patient-centered physical therapy in home settings. You will work under physician orders and with the Clinical Services team to promote recovery and independence in patients’ residences.

You should have a degree in physical therapy, state licensure, and preferred 2 years of home health experience, including familiarity with Medicare OASIS documentation. Join our supportive field team and make a meaningful impact.

Qualifications

• Must be licensed to practice physical therapy in the state.
• Degree in physical therapy from an accredited program.
• Two years of home health experience preferred.
• Knowledge of Medicare OASIS documentation.

Responsabilités

• Provide physician-prescribed physical therapy in patient homes.
• Improve or minimize residual physical disabilities.
• Restore patient to optimal function within capabilities.
• Collaborate with other home care staff in care planning.
• Direct and supervise personnel as required.
• Complete initial assessment, plan of care, and 30‑day reassessments.

A Physical Therapist (PT) administers physical therapy to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Director of Clinical Services/Clinical Supervisor or Manager.

What you’ll be doing:

• Provide physician prescribed physical therapy.
• Improve or minimize residual physical disabilities of the patient.
• Return the individual to optimum and productive level within the patient’s capability.
• Participate with all other home care personnel in patient care planning.
• Direct and supervise personnel as required.
• Responsible for initial assessment, plan of care, maintenance program development and modifications and reassessments every 30 days.

What we’re looking for:

• Graduate with a Bachelor’s, Master’s, or Doctorate in physical therapy from a physical therapist curriculum approved by the Commission on Accreditation in Physical Therapy Education, American Physical Therapy Association, or the Committee on Allied Health Education and Accreditation of the American Medical Association, or the Council of Medical Education of the American Medical Association and American Physical Therapy Association.
• Currently licensed in the state in which practicing
• Two (2) years’ experience preferred in a home health setting
• Knowledgeable and experience with Medicare OASIS documentation
