Job summary

Twin Lakes Rehabilitation and Nursing is seeking a PRN Physical Therapist (PT) to join our resident-centered care team. You will assess patients, establish treatment plans, and document progress under physician direction, coordinating with other disciplines.

Ideal candidates are licensed in the state and have graduated from an accredited PT program; geriatric/long-term care experience is preferred. We offer a positive work environment and competitive pay and benefits.

Qualifications

• Must be licensed as Physical Therapist in the state of practice.
• Graduation from an accredited Physical Therapy college or university.
• Geriatric or long-term care experience preferred.
• Experience with Case Mix Index and PPS scheduling preferred.

Responsibilities

• Assess referred patients requiring rehabilitation.
• Provide direct patient care to assess medical conditions and rehabilitation potential.
• Establish and administer a treatment program with goals per physician directions.
• Set realistic goals and educate patients and families in PT methods.
• Document condition, progress notes, discharge documentation, and recertifications.
• Collaborate with all disciplines to deliver patient-centered care.
• Perform duties assigned by Director of Rehabilitation.

Duties and Responsibilities of the Physical Therapist (PT):

• Responsible for the assessment of referred patients that require rehabilitation services.
• Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.
• Establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician.
• Set realistic and achievable goals for their patients
• Document and record the patients’ condition and educate patients and families in an appropriate physical therapy method.
• Evaluations, weekly progress notes, recertification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.

Requirements/Qualifications of the Physical Therapist (PT):

• Must have graduated from an accredited Physical Therapy college or university.
• Must have a valid professional license and registration for state of practice.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Experience with Case Mix Index and PPS scheduling is preferred however not limited to.
