Job summary

AvaMed Workforce in California is urgently seeking a full-time Physical Therapist for a correctional facility in Soledad, CA. The role offers 40 hours per week, a Monday-Thursday 7:00am-5:30pm schedule, and an orientation of two weeks.

Rate is $80/hr on 1099, with Travel/W2 options available. Under supervision, you will perform PT evaluations, develop treatment plans, and maintain documentation per facility policies.

Responsibilities

• Perform care within licensure scope and facility policies using evidence-based physical therapy practices.
• Provide PT evaluations on an as-needed basis as referred by the Institution designee.
• Deliver PT services to patients upon physician referral or direct access when applicable.
• Conduct an initial assessment and develop a treatment plan; communicate it to patient and medical staff.
• Complete reports and progress notes using approved documentation forms.
• Be available for telephone consultation while on duty.
• Maintain daily charting; delinquent charting may risk termination.

AvaMed Workforce is a Healthcare Staffing Agency that staffs all of California State Correctional Facilities. At the moment we are inURGENTneed of a Physical Therapist to take on a Full-Time assignment at the correctional facility located in Soledad, CA.

40 Hours per week (4/10's)

Location: Soledad, CA

Schedule: Monday-Thursday 7:00am-5:30pm

(Orientation 2 weeks, must be flexible)

Rate: $80/hr 1099 (Travel/W2 rate available)

Job Description:

Under the direction of the Institution designee, the Temporary/Relief Physical Therapist duties/responsibilities shall include, but are not limited to:

• a. Perform care consistent with the scope of licensure and in accordance with the Institution/Facilities policies and procedures and consistent with generally accepted evidence based Physical Therapy practices. Care provided must be sufficiently complex and consistent with the unique skills of a physical therapist;
• b. Provide Physical Therapy and evaluations on an "as needed" basis on patient/youth as pre-scheduled by designated Institution/Facility designee;
• c. Provide Physical Therapy services to patient/youth upon physician referral and order, or upon direct access to the Physical Therapy services in cases where the patient/youth is seeking Physical Therapy for an existing and documented diagnosis;
• d. Provide an initial assessment/evaluation on each referral and develop a treatment plan consistent with those findings and be responsible for communicating the treatment plan to the patient/youth and any relevant medical personnel;
• e. Reports or assessments/evaluations and progress notes shall be completed utilizing approved documentation forms in the patient's chart and/or outpatient health record;
• f. Vendor Worker shall be available for telephone consultation while on duty at the Institution/Facility;
• g. Maintain all charting on a daily basis. Any chart remaining delinquent for more than ten(10) days could result in termination of the Vendor Worker.
