Savannah Nursing & Rehabilitation in Savannah, TN is seeking a Full Time Physical Therapist to join our in-house rehab program. You’ll conduct screenings, develop care plans, and deliver skilled, medically necessary therapy aligned with regulatory standards.

Qualifications

• Must hold a valid Physical Therapist license.

Responsibilities

• Conduct screening and evaluation of patients with physical functioning disorders and/or wound care, and develop care plans.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions.
• Maintain safe patient handling with regular standing and walking and occasional lifting or repositioning.
• Perform procedures as part of the rehabilitation plan, including manual techniques and therapeutic exercises.
• Provide counseling to patients, families, and other providers regarding physical disorders.
• Prepare comprehensive discharge summaries in compliance with regulatory requirements.

We are a post-acute care facility with long-term care, short-term care, and geriatric services provided. We can offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery. We are currently seeking a Full Time Physical Therapist for our in-house rehab program!

Key Responsibilities

• Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans adhering to regulatory and clinical standards.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements.
• The role involves regular standing and walking throughout the facility and occasional lifting, transferring, and repositioning of patients and equipment. Physical demands vary throughout the day based on patient care needs.
• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.

Qualifications

• Physical Therapist license is required.
• Open to Physical Therapist at all experience levels. New Grads are welcome!
