Job summary

Millennium Physician Group in Sarasota County, FL is seeking a licensed Physical Therapist to join our Home Health Team. You will evaluate patients in their homes, develop individualized treatment plans, and deliver interventions to improve mobility, strength, balance and independence.

Qualifications

We are seeking a Physical Therapist (PT) to join our Home Health Team in Sarasota County. In this role, there is a real impact in the comfort of patients’ homes. As part of our compassionate, high‑performing care team, you will evaluate patients, develop individualized treatment plans, and provide therapeutic interventions to improve mobility, strength, balance, and functional independence. You will collaborate with an interdisciplinary care team, educate patients and families on rehabilitation and injury prevention strategies, maintain accurate clinical documentation, and ensure compliance with all regulatory standards to deliver high-quality patient care and achieve optimal outcomes.

How will you make an impact & Requirements

What You’ll Do

• Patient Assessment: Conduct comprehensive evaluations of patients' physical abilities, limitations, and functional needs to accurately diagnose impairments and develop individualized treatment plans.
• Therapeutic Interventions: Implement a variety of therapeutic techniques, including manual therapy, therapeutic exercises, and gait training to improve patients' mobility, strength, balance, and pain management.
• Patient Education: Educate patients and their families on proper body mechanics, home exercise programs, and injury prevention strategies to promote long-term health and well‑being.
• Collaboration: Work collaboratively with physicians, nurses, occupational therapists, and other healthcare professionals to develop comprehensive care plans and ensure optimal patient outcomes.
• Documentation: Maintain accurate and timely documentation of patient evaluations, treatment plans, progress notes, and discharge summaries.
• Compliance: Adhere to all relevant regulatory standards, including HIPAA and Medicare regulations.

What We’re Looking For

• Licensure: Current and valid Physical Therapist license in the state of Florida.
• Experience: Minimum of 1 year of clinical experience as a Physical Therapist, preferably in a home health or a related setting.
• Skills: Strong clinical skills, excellent communication and interpersonal abilities, and a patient‑centered approach to care.
• Professionalism: Strong work ethic, reliability, and a commitment to providing high‑quality care.
• Transportation: Reliable transportation to visit patients in their homes.

Compensation Range:

$77,417.00

to

$116,126.00

The anticipated base salary range represents the Company's good‑faith estimate of the compensation it reasonably expects to pay for this position at the time of posting. Actual compensation will be determined based on factors including experience, skills, qualifications, geographic location, internal equity, and business needs.
