Responsibilities

What you will do: Responsibilities include but are not limited to:

• Perform evaluations and develop treatment plans
• Regularly re-assess the effectiveness of treatment plans, attend staffing meetings and family conferences
• Perform discharge evaluations, make appropriate safety recommendations, and provide referral and education resources
• Communicate patient’s needs and progress to the treatment team, physician, patient, and family members
• Supervise Physical Therapy Assistants as needed
• Collaborate with other disciplines to ensure coordination of care and improve patient outcomes

Qualifications

What you will need:

• Bachelor’s degree in Physical Therapy from an accredited program
• Current, unrestricted Physical Therapist license in the practicing state
• Current Basic Life Support or CPR certification as per state regulations
• Familiarity with assisted living, independent living, geriatrics, and/or home health
• Knowledge of evidence-based therapy interventions
• Effective and professional communication skills in verbal and written form
• Ability to lift up to 50 pounds; may involve lifting up to 100 pounds following safety procedures
• Physical requirements include lifting, moving, standing, walking, pushing, pulling, bending, reaching, kneeling, and positioning
• A health screen or examination may be required prior to and periodically during employment to verify physical capability
