Job Title: Physical Therapist – Home Health

Location: Supporting strictly within Santa Rosa County California

Schedule: Monday–Friday, 8:30 AM – 5:00 PM PT

Employment Type: Full-Time, Permanent (Part-Time/Contract is also available)

Guaranteed Hours: 40 hours/week

Compensation: $120,000 – $150,000 annually

Start Date: ASAP

About the Role

We are seeking a licensed Physical Therapist to join our Home Health team in Santa Rosa, CA. This is a highly autonomous role where you’ll manage your own schedule and provide exceptional care to patients within Santa Rosa County.

Key Responsibilities

• Deliver in-home physical therapy services to patients in Santa Rosa County.
• Manage a caseload of approximately 30 points per week (5–6 patients/day).
• Utilize provided iPad and AI-powered tools for efficient charting and documentation.
• Complete documentation on time to qualify for additional incentive pay.
• Travel within a 20–25 mile radius; mileage reimbursement provided.

Must-Have Qualifications

• Active Physical Therapist (PT) license in the state of California.

Preferred Qualifications

• Prior experience in home health care.
• Familiarity with Kinnser EMR (WellSky) system.
