Job summary

Responsibilities

Responsibilities:

• Evaluate and assess patients' physical conditions, functional limitations, and rehabilitation needs
• Develop and implement individualized treatment plans to improve mobility, strength, and function
• Administer therapeutic exercises, manual therapy, and other evidence-based PT interventions
• Monitor and document patient progress, adjusting treatment plans as needed
• Collaborate with interdisciplinary care teams including physicians, occupational therapists, and caregivers
• Educate patients and families on home exercise programs, injury prevention, and safe mobility techniques
• Maintain accurate and timely clinical documentation in compliance with regulatory standards

Requirements:

• Valid California Physical Therapist (PT) license in good standing
• Graduate degree (DPT or MPT) from an accredited physical therapy program
• Experience working with geriatric or senior populations preferred
• Strong knowledge of rehabilitative techniques, therapeutic modalities, and functional mobility training
• Excellent communication, interpersonal, and patient education skills
• Ability to work independently as well as collaboratively within a care team
• Current CPR/BLS certification required
