Responsibilities

What you will be doing

• Conduct Physical Therapy assessments to determine the Physical Therapy care needs, preferences and goals of the participants and actively participate in IDT meetings to develop participant care plans.

• Deliver and document Physical Therapy interventions in accordance with physician orders and/or as agreed upon in the participants’ care plans including but not limited to providing active and passive exercise, muscle re‑education, gait and functional training of ADLs; treatments such as massage, heat, sound, water light or electricity.

• Supervises the Therapy Assistants including providing training, instruction and guidance needed to implement Functional Maintenance Programs (FMP).

• Provide training and instructions to staff, participants and caregivers/families in the use and care of therapeutic equipment.

• Order, track and manage maintenance for equipment.

• Remain alert to pertinent input from other team members, participants, and caregivers and update the IDT promptly of any changes in participants’ condition or functional status.

• Work with the other members of the IDT to support smooth care transitions between settings (e.g., hospitals, skilled nursing facilities, home, etc.)

• In coordination with the Marketing Team, support enrollment of prospective participants into the program.

• Participate in end‑of‑life care coordination and support.

• Manage direct reports including training, supervising and mentoring.

• Be legally authorized to practice in position location, required.

• Only act within the scope of his or her authority to practice.

• Complete all required documentation in a timely and accurate manner.

• Protect privacy and maintain confidentiality of all company procedures and information about team members, participants, and families.

• Follow company policies and procedures and participate in any required Quality Improvement activities, staff training and meetings.

• Communicate regularly with Supervisor and team regarding workload and priorities.

• Timely completion of all mandated trainings and education.

• Timely completion of all mandated occupational health screenings as needed.

• Exercises flexibility in performing assignments as business needs evolve.

What you must have

• Bachelor's Degree in Physical Therapy. The qualifying Physical Therapy Degree earned outside of the US must be evaluated to the US equivalent to a Degree in Physical Therapy.

• Minimum of two years’ Physical Therapy experience in a clinical setting with a frail or elderly population.

• Physical Therapy knowledge and skills necessary to treat frail, elderly participants with complex needs.

• Highly motivated, self‑directed, able to execute tasks in a quickly changing environment and make sound decisions in emergency situations.

• Two years’ supervisory experience with demonstrated ability to mentor and develop team members

• Ability to process accurate information in a timely manner.

• Excellent leadership, organizational and communication skills in settings with seniors, their families and interdisciplinary team member

• Experience leading in a data‑driven organization, leveraging reports and data to prioritize and manage people and projects.

• Highly motivated, inspirational leader who is able to innovatively engage, support and remove barriers for the team to ensure the team consistently meet established goals.

• Excellent clinical, organizational and communication skills in settings with seniors, their families and interdisciplinary team members.

• Ability to work independently with minimal supervision

Salary/Rate Range: $65.00-$70.00/hour W2

Other Information

• M-F 8a-4:30p

• Combination of on‑site and mobile depending on participants needs.

• 1-2 days dedicated to home visits for home safety and therapy needs. PACE setting, PT will be involved in interdisciplinary meeting, participating in care coordination besides direct patient care.

• Must have reliable transportation.
