Job summary

Midtown Oaks Post Acute in Sacramento, CA is hiring a Physical Therapist (PT) to join our rehabilitation team in Sacramento, CA. We welcome both experienced clinicians and new graduates.

In this role, you will provide skilled physical therapy services to residents recovering from illness, injury, and surgery in a skilled nursing and post-acute setting. We offer $65.00 - $70.00 per hour pay, a $7,500 sign-on bonus for full-time employees, flexible scheduling, mentorship and clinical support, and

Qualifications

• Active California Physical Therapist license or eligibility to obtain licensure.
• Strong clinical reasoning and communication skills.
• Strong documentation and time-management skills.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Deliver evidence-based physical therapy services in a skilled nursing and post-acute setting.
• Establish functional goals to help patients regain mobility and independence.
• Complete evaluations, progress notes, discharge summaries, and required documentation.
• Collaborate with physicians, nursing staff, families, and interdisciplinary team members.
• Educate patients and caregivers on treatment plans and home exercise programs.
• Support positive patient outcomes and satisfaction.
