Qualifications

• Experience practicing physical therapy in home health or outpatient rehabilitation settings.
• Background in outpatient clinics, inpatient hospital, or rehabilitation centers.
• Experience treating geriatrics, neurology, orthopedics, and post-surgical rehabilitation.
• Familiarity with EHR systems for documentation and patient tracking.
• Ability to develop treatment plans for musculoskeletal and neurological conditions.
• Excellent communication to motivate patients and educate families.
• Experience with discharge planning and multidisciplinary teamwork.

Responsibilities

Experience

• Proven experience practicing in a physical therapy role within home health or outpatient rehabilitation settings.
• Strong background in outpatient clinic experience, inpatient hospital experience, or outpatient rehabilitation centers.
• Familiarity with treating diverse populations including geriatrics, neurology cases, orthopedics, and post-surgical rehabilitation.
• Experience working with electronic health records (EHR) systems such as EMR systems for documentation and patient tracking.
• Demonstrated ability to develop treatment plans for various conditions including musculoskeletal injuries and neurological disorders.
• Excellent client communication skills to motivate patients and educate families about care plans and home & community care strategies.
• Prior experience with discharge planning processes and working collaboratively with multidisciplinary teams to ensure comprehensive patient care.
