Apex Home Health, Hospice & Home Care is seeking a licensed Physical Therapist to join our outpatient rehab team in Rockford, Illinois. You will evaluate patients with musculoskeletal, neurologic, and post-surgical conditions and develop individualized treatment plans.

You will deliver hands-on therapy to improve function, mobility, and independence, collaborate with a multidisciplinary team, educate patients and families, and document care in our EMR system; mentorship and continuing education
