Job summary

Santa Fe Care seeks a full-time Physical Therapist (PT) for our Skilled Nursing Facility. You will evaluate patients, create individualized plans, and provide evidence-based interventions to improve mobility and independence, while collaborating with the care team and documenting per regulations.

Qualifications

• Valid New Mexico state license as a Physical Therapist (PT).
• Degree in Physical Therapy from an accredited program.

• Strong communication skills and a patient-centered care approach.

Responsibilities

• Conduct thorough PT evaluations and develop individualized treatment plans
• Deliver evidence-based therapeutic interventions to improve mobility, strength, and functional independence
• Document all patient care in accordance with state, federal, and facility guidelines
• Collaborate with nursing, therapy, and medical teams to ensure coordinated care
• Educate residents and family members on exercise programs, safety, and functional strategies
• Participate in care conferences and contribute to ongoing plan-of-care updates

Position Overview

We are seeking a Full-Time Physical Therapist to provide high-quality rehabilitative care within our skilled nursing facility. Key Responsibilities
