Job Title: Home Health Physical Therapist

Location: Pulaski, TN

Job Type: Part Time

Position Overview

The Physical Therapist is responsible for the assessment and evaluation of patient care needs related to functional status/mobility, endurance/stamina, pain and other medical conditions. Based on the assessment, the Physical Therapist determines a treatment plan, performs interventions aimed at improving the patient's well‑being, and evaluates progress.

Responsibilities
  • Evaluate the patient's functional status and physical therapy needs.
  • Assist the physician in developing the therapy plan of care.
  • Observe and report the patient's response to treatment and any changes in condition to the supervising nurse and/or physician.
  • Treat patients to relieve pain, develop and restore function, and/or maintain maximum functioning using acceptable standards of practice.
  • Instruct and inform patients, caregivers, and the health‑care team regarding the plan of care, functional limitations, and progress toward goals.
Qualifications
  • Current PT license in the state where services are rendered.
  • Current CPR certification, driver’s license, and vehicle insurance with access to a dependable vehicle or public transportation.
  • Ability to reliably commute to multiple patient residences daily within the geographic area assigned.
