Pinnacle Nursing & Rehabilitation Center in Price, UT is hiring a full-time Physical Therapist to support our in-house rehab program and geriatric care. Join a stable therapy team focused on post-acute recovery and exceptional patient outcomes.

The role covers screening, evaluation, treatment, care planning, and comprehensive discharge summaries. A current Physical Therapist license is required; new grads are encouraged to apply and will be supported by our CAPLICO values-driven culture.

Qualifications

Responsibilities

• Conduct screening and evaluation of patients with physical functioning disorders, developing care plans adhering to standards.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions.
• Administer procedures as part of rehabilitation, including manual techniques, ambulation, therapeutic exercises, modalities, and wound care.
• Provide consultation to patients, families, and other providers regarding physical disorders.
• Generate discharge summaries ensuring regulatory compliance.

Pinnacle Nursing & Rehabilitation in Price, UT is a post-acute care facility with long-term care, short-term care, and geriatric services provided. We can offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery. We are currently seeking a full time Physical Therapist for our in-house rehab program!

Key Responsibilities

• Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans adhering to regulatory and clinical standards.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements.
• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.

Qualifications

Physical Therapist license is required.
