Qualifications

Now Hiring: Physical Therapist (PT) – Home Health. Ventura County, CA – Ventura, Oxnard, Valencia & Surrounding Areas. Per Diem / PRN | Part-Time | Full-Time Opportunities Available. Join Remedy Home Health Care — Where Expert Care Meets Community. Remedy Home Health Care is actively seeking a licensed Physical Therapist (PT) to join our growing Home Health team in Ventura County. This Per Diem Physical Therapist role offers flexibility, autonomy, and the opportunity to provide high-quality rehab and community-based care directly to patients in their homes.

As a Home Health Physical Therapist, you will work with senior care populations, including geriatric patients recovering from illness, injury, or surgery. You’ll deliver skilled therapy services in home-based, assisted living, and community care settings, helping patients regain independence and improve their quality of life.

Responsibilities

• Conduct in-home physical therapy evaluations and reassessments
• Develop and implement personalized rehabilitation care plans
• Improve strength, balance, mobility, and functional outcomes
• Provide therapy in home health, assisted living, and community care settings
• Educate patients and families on fall prevention and safe mobility techniques
• Document visits using WellSky (Kinnser) EMR
• Collaborate with interdisciplinary home health teams

Qualifications

• Licensed Physical Therapist (PT) – California (Required)
• CPR Certification (AHA approved)
• Minimum 1 year of clinical experience (Home Health preferred)
• Experience in senior care, geriatrics, rehab, or community-based care preferred
• Strong clinical, communication, and patient education skills
