Job summary

Palms-Home-Care is seeking a dedicated Physical Therapist Assistant (PTA) to join our healthcare team under the supervision of a licensed Physical Therapist. The PTA will deliver high-quality, patient-centered care and support rehabilitative goals in home health and outpatient settings.

Responsibilities include implementing treatment plans, guiding exercises, documenting progress, educating patients and families, and collaborating with RNs, PTs, OTs, and Speech Therapists to ensure holistic care

Qualifications

• Current state licensure as a Physical Therapist Assistant.
• Excellent communication and interpersonal skills for effective collaboration with patients, families, and healthcare professionals.
• Ability to work collaboratively within an interdisciplinary care team.
• Solid understanding of anatomy, physiology, and medical terminology.

Responsibilities

• Implement treatment plans designed by the supervising Physical Therapist, ensuring effective, individualized care.
• Administer therapeutic exercises and modalities for patients in geriatrics, orthopedics, pediatrics, outpatient, and acute care settings.
• Observe, assess, and document patient responses and progress, reporting changes or concerns promptly to the supervising PT.
• Educate patients and families on home exercise programs, safety precautions, and techniques to promote continued recovery.
• Maintain accurate and thorough treatment records, progress notes, and other required documentation.
• Collaborate with multidisciplinary team members—including RNs, PTs, OTs, and Speech Therapists to ensure coordinated, holistic care.
• Promote and maintain a safe, clean, and supportive environment for all patients.
