Fresh Pond Physical Therapy is seeking a dedicated Physical Therapist to join our team in Queens/Brooklyn, New York. The role involves evaluating, diagnosing, and treating a wide range of conditions, with personalized plans and ongoing progress monitoring.

You will work in a collaborative clinical setting, delivering evidence-based interventions and maintaining high standards of care. Applicants should have a PT degree, NY licensure eligibility, and strong communication skills.

Qualifications

• Bachelor’s Degree in Physical Therapy (required).
• Master’s or Doctorate Degree in Physical Therapy (preferred).
• Current or eligible for New York State Physical Therapy licensure.
• Prior clinical experience preferred, especially in outpatient settings.
• Strong communication, organizational, and interpersonal skills.

Responsibilities

• Conduct comprehensive patient evaluations through observation and assessment.
• Develop and implement individualized treatment plans based on clinical findings.
• Deliver therapeutic interventions using physical and mechanical techniques as prescribed.
• Monitor patient progress and modify treatment plans as necessary.
• Maintain accurate, timely, and compliant patient documentation.
• Collaborate with a team of therapists to enhance patient outcomes, engagement, and retention.
• Participate in quality assurance activities, including chart reviews.
