Morris Heights Health Center is seeking a compassionate Physical Therapist to join our multidisciplinary care team in a community-based setting. The role focuses on evaluating and treating patients across the lifespan to improve mobility, reduce pain, and restore function.

The ideal candidate has NY licensure and 2+ years of outpatient or related experience, with a patient-centered approach, strong documentation, and teamwork.

Formación

• Graduate of an accredited Physical Therapy program.
• Current New York State Physical Therapist licensure/registration.
• Two or more years of Physical Therapist experience preferred.

Responsabilidades

• Perform comprehensive physical therapy evaluations and interpret findings to determine the nature and extent of functional limitations.
• Develop individualized treatment plans and measurable therapeutic goals.
• Provide direct physical therapy treatment and modify treatment plans based on patient progress and clinical findings.
• Coordinate care with physicians, advanced practice providers, nursing, and other members of the healthcare team as appropriate.
• Educate patients and families regarding treatment plans, therapeutic exercises, mobility, injury prevention, and ongoing wellness.
• Assess patient progress toward established goals and determine when treatment modifications are necessary.
• Initiate discharge planning and appropriately discharge patients once maximum therapeutic benefit has been achieved.
• Maintain accurate and timely documentation of physical therapy services and patient management.
• Provide appropriate clinical direction and oversight to Physical Therapist Assistants and Physical Therapy Aides.

Morris Heights Health Center (MHHC) is seeking a compassionate, clinically strong, and patient-centered Physical Therapist to join our multidisciplinary care team. This is an opportunity to work in a community-based healthcare environment where physical therapy is an integral part of helping patients improve mobility, reduce pain, restore function, prevent disability, and maintain independence. The Physical Therapist provides direct patient care across the lifespan, evaluating patients with medical or health-related conditions that affect movement and functional activity and developing individualized treatment plans to help patients achieve meaningful functional goals.

What You’ll Do

What We’re Looking For

Education

• Graduate of an accredited Physical Therapy program.
• Bachelor’s, Master’s, or Doctoral degree in Physical Therapy as applicable to professional training and licensure requirements.

Licensure

• Current New York State Physical Therapist licensure/registration required.

Experience

• Two or more years of Physical Therapist experience preferred.
• Outpatient, ambulatory care, community health, or multidisciplinary healthcare experience is highly desirable.
