Job summary

Hands of Hope Physical Therapy in Brooklyn, NY seeks a licensed Physical Therapist for outpatient care to assess, plan and implement rehabilitation programs that improve mobility and reduce pain. The PT will collaborate with the pain management department to ensure comprehensive patient care and optimal outcomes.

You will conduct assessments, develop individualized plans, educate patients, document progress, and participate in continuing education to stay current with evidence-based practice.

Qualifications

  • Licensed Physical Therapist in New York state.
  • Current CPR/BLS certification preferred.

Responsibilities

  • Conduct comprehensive assessments to evaluate patients' physical conditions and mobility.

Job Summary

Assess, plan, organize, and participate in rehabilitative programs that improve mobility, relieve pain, increase strength, and improve or correct disabling conditions resulting from disease or injury. The physical therapist will collaborate closely with the pain management department to ensure comprehensive patient care and optimal treatment outcomes.

Primary Job Duties

1. Conduct comprehensive assessments to evaluate patients' physical conditions, functional abilities, and mobility limitations.

2. Develop individualized treatment plans based on assessment findings, patient goals, and clinical reasoning.

3. Implement therapeutic techniques and modalities, including therapeutic exercises, manual therapy, electrical stimulation, ultrasound, and heat/cold therapy.

4. Educate patients and their families on treatment goals, home care techniques, and injury prevention strategies.

5. Maintain accurate and detailed patient records, progress notes, and treatment plans in compliance with regulatory standards and organizational policies.

6. Collaborate with interdisciplinary healthcare teams, including physicians, nurses, and the pain management department, to coordinate patient care and achieve optimal outcomes.

7. Perform periodic reassessments to evaluate patient progress toward treatment goals and modify interventions as necessary.

8. Engage in continuing education and professional development activities to stay current with advances in physical therapy practice and evidence-based treatments.

9. Adhere to ethical standards and legal regulations governing physical therapy practice, ensuring patient confidentiality and professional conduct.

Qualifications
  • Bachelor's, Master's, or Doctoral degree in Physical Therapy.
  • Current state licensure as a Physical Therapist.
