Job summary

CommonSpirit Health is seeking a dedicated Physical Therapist to provide compassionate, in-home care to patients with injuries, chronic illness, and other medical needs. You will evaluate and treat disabilities, injuries and disease using manual therapy, modalities, and exercises, while educating patients and families.

You will collaborate with a multidisciplinary team, supervise assistants, complete timely documentation, and participate in in-services and care coordination.

Qualifications

• Degree in physical therapy from an accredited program.
• Licensed to practice physical therapy in MN.
• Two years of relevant PT experience.
• Current CPR certification.
• Valid MN driver’s license.

Responsibilities

• Provide patient care including evaluation and treatment as referred by providers.
• Provide supervision for Physical Therapist Assistants and support personnel.
• Complete documentation related to patient care in a timely manner.
• Participate in in-services for staff and providers.
• Collaborate with the healthcare team and provide professional recommendations regarding patient care.
• Participate in patient-related care coordination activities.

Job Summary and Responsibilities

As our Physical Therapist, you will provide compassionate, personalized care in the home. Our skilled staff improve lives by providing care for our patients wherever they call home. We are focused on caring for patients recovering from injuries, living with a chronic illness, and other medical needs.

Every day, you will evaluate and treat disabilities, injuries and disease using a range of manual therapy, modalities and exercises as well as providing education and consulting services. The Physical Therapist complies with all regulatory, professional, ethical, quality improvement and facility standards, performs charting and patient progress notes as well as supervision, training and assistance to department personnel. The Physical Therapist serves in a multidisciplinary capacity providing services to a wide variety of populations which may include pediatric, medical/surgical, orthopedics, outpatient. Homecare services are also provided and may increase travel frequency.

You should be personable, positive, and motivated to provide quality care to a wide variety of patients every day.

Job Requirements

Required

• Possesses a degree from a baccalaureate, Masters or Doctorate program in physical therapy approved by an accredited organization


• Licensed to practice as physical therapist within the State of MN


• Two years of appropriate experience as a physical therapist


• Possesses and maintains current CPR certification


• Current MN Driver’s license

Preferred

• Community/home health experience
