Job summary

Blue Stone Therapy in Minneapolis seeks a Physical Therapist to join our rehabilitation team at Mount Olivet Care Home. You will work with an interdisciplinary group to provide PT services to adult and geriatric residents in a skilled nursing and post-acute setting.

This full-time or PRN role offers clinical autonomy with supportive leadership, meaningful one-on-one patient relationships, and growth opportunities.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Physical Therapist license (or eligibility for licensure) in Minnesota.

Responsibilities

• Evaluate patients and develop individualized plans of care.
• Deliver evidence-based physical therapy focused on mobility, balance, strength and independence.
• Collaborate with physicians, nursing staff, families, and team members for optimal outcomes.
• Supervise Physical Therapy Assistants in accordance with state licensure requirements.
• Complete timely documentation to support quality patient care and regulatory compliance.
