Responsibilities

  • Assumes responsibility for screening, evaluation, and treatment of facility patients/residents needing Physical Therapy services.
  • Reviews medical information and conducts screenings to determine need for services.
  • Performs patient evaluations under physician order and develops a treatment plan.
  • Implements treatment plan and supervises therapy staff.
  • Participates in continuing education for professional growth.
  • Maintains patient confidentiality and accurate billing information.
  • Attends interdisciplinary meetings and family meetings as appropriate.
  • Adheres to documentation requirements of the Rehab department.
  • Supervises Physical Therapy Assistants and rehabilitation staff as needed.

Job description

The Physical Therapist must: have a Bachelor and/or Master's and/or Doctorate Degree in Physical Therapy, have a current state license, have excellent oral and written communication skills, demonstrate competency in therapy treatment and evaluation principles, techniques, materials, and equipment utilization, and demonstrate knowledge of state and federal accreditation standards.

The candidate must also have availabiltiy to complete evaluations on new admissions over the weekend in a busy skilled nursing facility typically getting 5-10 admissions between Friday and Sunday.

The Physical Therapist:
  • Assumes responsibility for the screening, evaluation, and treatment of facility patients/residents needing Physical Therapy services
  • Reviews medical information and conducts screenings prior to evaluations to determine patient need for services and ability to benefit from skilled services
  • Performs patient's evaluations under physician order and develops a treatment plan in accordance with patient's needs
  • Implements treatment plan through direct treatment, education of treatment staff, and supervision of physical therapy staff
  • Assumes responsibility for continued professional growth and development by participating in continuing education programs and other appropriate learning experiences
  • Maintains confidentiality of all patient, facility, and physician-related information
  • Maintains accurate billing information, in accordance with procedures, on a daily basis
  • Attends and participates in interdisciplinary meetings, as appropriate, to relay clinical findings and recommendations to other members of the rehab team
  • Attends and participates in family meetings, as appropriate
  • Adheres to documentation requirements of the Rehabilitation department and the facility
  • Conducts staff training/education, as appropriate, via formal and informal in-services and consultations
  • Reviews and follows the policies and procedures outlined by the facility and Rehab Policy and Procedure Manual
  • Supervises Physical Therapy Assistants and rehabilitation aides/extenders as indicated by patient needs and state laws and discipline practice act
  • Supervises non-professional staff, as appropriate
