Job summary

Paycom - ATS is seeking a licensed Physical Therapist in Arizona to evaluate referred patients and implement individualized treatment plans. Responsibilities include documenting visits in the EHR, tracking progress toward goals, and participating in weekly multidisciplinary rounds to refine each patient’s plan of care.

The role requires a Master of Physical Therapy, AZ licensure, and strong interpersonal skills.

Qualifications

• Must hold Arizona Physical Therapist license.
• Master of Physical Therapy degree required.
• Understanding of substance and mental health issues.
• Interpersonal and communication skills required to interact with patients, physicians and health team members.
• Computer proficiency to access, enter, and interpret data in the EHR.

Responsibilities

• Evaluate and treat each physician-referred patient.
• Design and document a treatment plan based on the evaluation results.
• Document each visit in the EHR and track progress toward goals.
• Attend weekly multidisciplinary rounds to discuss plan of care and goals.
• Discharge summary when goals are attained or per patient wishes.

Job description

Summary of Position:Each patient that is referred to the Physical Therapy Program by a CTC physician will receive a complete evaluation by a registered Physical Therapist. Each patient will then receive treatment according to an individualized treatment plan. All services are to be recorded in the Electronic Health Record (EHR).Essential Duties:Clinical ResponsibilitiesEvaluate and treat each physician referred patient.Design and document a treatment plan based on the results of the evaluation.Document each visit in the Electronic Health Record (EHR).Document progress made toward treatment goals.Complete a discharge summary when goals are attained or discharged according to the wishes of the patient.Attend weekly multi-disciplinary rounds; participate in the discussion surrounding patients plan of care, goals and continued therapy.Other responsibilities/accountabilities:Duties and responsibilities may be added, deleted or changed to meet program needs, at the discretion of the Medical Director.Qualifications:Basic Knowledge/Skills:Understanding of substance and Mental Health issuesInterpersonal and communication skills sufficient to effectively interact with patients, physicians, and health team members requiredComputer proficiency inclusive of ability to access, enter, and interpret computerized data/information requiredBasic Life Support certification required (BLS)Experience and Education:License to practice physical therapy in Arizona requiredMasters of Physical Therapy required.Physical and Mental RequirementsDemonstrated interest in working with and underserved populationPerforms related functions as required.We are an equal-opportunity employer.
