Job summary

Eastern Health Group is seeking a Physical Therapist (PT) to assess referred patients requiring rehabilitation and provide direct patient care in our resident-centered facilities in Virginia. The role includes establishing and administering individualized treatment plans, documenting progress, and coordinating with interdisciplinary teams under Rehabilitation leadership.

Qualified applicants will have a graduated PT degree, a valid license, and experience in geriatric or long-term care settings

Qualifications

• Must graduate from an accredited Physical Therapy program.
• Must hold a valid PT license and license in the state of practice.

Responsibilities

• Assess referred patients requiring rehabilitation.
• Provide direct patient care to assess medical condition, functional capabilities, limitations and potential for rehabilitation.
• Establish and administer a treatment program with goals according to patient capacity and tolerance under physician direction.
• Set realistic and achievable goals for patients.
• Document and educate patients and families in a physical therapy method and maintain progress notes.
• Perform evaluations, weekly progress notes, recertification and discharge documentations.
• Collaborate with all disciplines to provide patient-centered care.
• Perform all other duties assigned by Director of Rehabilitation.
• Maintain confidentiality, HIPPA and Federal Residents Rights.

Education

Graduate of accredited Physical Therapy program

Valid PT license/registration in state of practice

Duties And Responsibilities

• Responsible for the assessment of referred patients that require rehabilitation services.
• Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.
• Establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician.
• Set realistic and achievable goals for their patients
• Document and record the patients’ condition and educate patients and families in an appropriate physical therapy method.
• Evaluations, weekly progress notes, recertification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.

Requirements/Qualifications

• Must have graduated from an accredited Physical Therapy college or university.
• Must have a valid professional license and registration for state of practice.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Experience with Case Mix Index and PPS scheduling is preferred however not limited to.
