Southland Rehabilitation & Healthcare in Lufkin, TX is seeking a PRN Physical Therapist to join our in-house therapy team. You will conduct screenings/evaluations, develop care plans, and deliver rehabilitation interventions aligned with standards.

Licensed Physical Therapist with flexibility for PRN hours. We value CAPLICO principles and patient-centered care, and offer a supportive environment with ongoing opportunities for professional growth.

Responsibilities

• Screen and evaluate patients with physical functioning disorders to develop care plans.
• Deliver rehabilitation interventions that are skilled, medically necessary, and compliant with regulatory standards.
• Administer procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, devices, and wound care.
• Provide consultation and counseling to patients, families, caregivers and other service providers regarding physical disorders.
• Generate comprehensive discharge summaries of services provided, ensuring regulatory compliance.

PHYSICAL THERAPIST - PRN - SOUTHLAND REHABILITATION & HEALTHCARE - LUFKIN, TX

Southland Rehabilitation and Healthcare is a post-acute care facility with long-term care, short-term care, and geriatric outpatient services provided. We can offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery. We are seeking a PRN Physical Therapist to join our in-house therapy team.

Key Responsibilities

• Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans adhering to regulatory and clinical standards.


• Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements.


• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.


• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.


• Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.
