Stanford Health Care – Tri-Valley in Livermore, CA is seeking a licensed physical therapist to assess patients, design and implement individualized treatment plans, and collaborate with the care team.
Responsibilities include initial evaluations, documentation, discharge planning, patient education, and adherence to CI-CARE standards. Requires CA PT license, BLS, and 3–5 years of experience.
Assesses patients and establishes appropriate treatment plan based upon diagnosis and professional standards. Provides appropriate treatment of patients using specific modalities and treatment techniques. Accurately documents treatments and care of patients and communicates with other team members. Participates in individual and department continuous improvement and education activities.

Graduation from a physical therapy program from an accredited college or university (Required).
3+ to 5 years of experience (Required).
