  • Evaluates effectiveness of treatment at various stages and adjusts as needed to maximize benefit.
  • Educates patients on device use and optimal outcomes; ensures proper fit.
  • Supervises PTA, aides, students and volunteers as schedule allows.
  • Participates in team and hospital meetings, committees and initiatives.

Overview

Assesses patients and establishes appropriate treatment plan based upon diagnosis and professional standards. Provides appropriate treatment of patients using specific modalities and treatment techniques. Accurately documents treatments and care of patients and communicates with other team members. Participates in individual and department continuous improvement and education activities.

Responsibilities
  • Direct patient care, including the assessment and treatment of patients, developing and carrying out individually designed treatment programs.
  • Performs and documents an initial evaluation, interprets data to identify problems, and determines functional limitations prior to intervention.
  • Performs ongoing documentation in the patient medical record with functional limitations, goals and appropriate timelines.
  • Communicate with referring physician to obtain certification of plan of care and document as appropriate.
  • Include the patient/caregiver in the treatment process, evaluate effectiveness and discharge as necessary.
  • Evaluate the effectiveness of treatment at various stages and make adjustments to achieve maximum benefit or discharge when treatment is no longer needed.
  • Ensure patient education, proper fit and use for assistive devices as necessary.
  • Supervise PTA, PSM Aide, students and volunteers as the clinical schedule dictates.
  • Participate in team and hospital-wide meetings, committees and initiatives as assigned.
  • Assist with department finances with accurate daily billing practices, active daily maintenance of patient care schedule.
  • Perform duties in accordance with the C-I-CARE Standards of the Hospital, which underpin patient-centered interactions.
Education Qualifications

Graduation from a physical therapy program from an accredited college or university (Required).

Experience Qualifications

3+ to 5 years of experience (Required).

Knowledge, Skills and Abilities
  • Good physical condition with ability to lift equipment and patients moderately.
  • Ability to work flexible schedules and varying locations as assigned.
  • Strong verbal and written English communication skills across all hospital levels and departments.
  • Strong organization and time management abilities.
  • Problem solver and critical thinker with the ability to anticipate issues.
  • Strong computer skills.
  • Self-initiative and knowledge of Medicare documentation standards.
  • Knowledge of hospital safety, quality and Joint Commission standards.
  • Knowledge of standard medical abbreviations for documentation.
Licenses and Certifications
  • Current American Heart Association Certification for Basic Life Support for Healthcare Providers (required).
  • Physical Therapy licensure in the State of California (required).
  • Codes: OTR, PT, SLP (as applicable).
