Job summary

Avir at Kirbyville in Kirbyville, TX seeks a Full-Time Physical Therapist to join our in-house team delivering person-centered care for residents. You will collaborate with clinicians to implement individualized rehabilitation plans and support ongoing treatment goals.

The ideal candidate holds a Texas PT license, demonstrates current treatment knowledge, and can work with diverse patient populations and families in a long-term care setting.

Qualifications

• Experience in SNF/long-term care settings preferred.
• Up-to-date knowledge of physical therapy treatments.
• Familiarity with Net Health Electronic Documentation System preferred.
• Ability to work with patients with diverse personalities.

Responsibilities

• Consulting with patients to understand their physical condition.
• Assessing patient evaluations and test results.
• Developing treatment plans using various techniques.
• Creating personalized rehabilitation programs.
• Administering prescribed PT treatments to relieve pain and restore function.
• Ensuring all treatments align with the plan of care.
• Monitoring patient progress and adjusting care as needed.
• Providing education to patients and families.
• Collaborating with the interdisciplinary team for quality care.
• Accurately documenting care per regulations.
• Complying with all rules and procedures.
• Attending meetings as designated by Rehab Director.

Physical Therapist Responsibilities:

• Consulting with patients to learn about their physical condition.
• Assessing and interpreting patient evaluations and test results.
• Developing treatment plans using a variety of treatment techniques.
• Creating personalized rehabilitation programs for patients.
• Administering medically prescribed physical therapy treatments to relieve pain, improve or restore function, and promote healing.
• Ensure all treatment delivered is in accordance with the established plan of care.
• Monitor patient response to treatment intervention and modify treatment approach and directive as clinically appropriate with IDT collaboration.
• Provide clinical support and education to patients, family members, and caregivers.
• Collaborate with other members of the interdisciplinary team to ensure quality patient care is provided.
• Ensure documentation is completed thoroughly and timely and complies with all company and state/federal regulatory requirements.
• Complying with all rules, regulations, and procedures.
• Attend required meetings as designated by the Director of Rehab.

Physical Therapist Skills:

• SNF/Long term care experience preferred.
• Current knowledge of treatment practices
• Knowledge of Net Health Electronic Documentation System preferred.
• Ability to manage patients with different types of personalities.

Physical Therapist Requirements:

• Active/Valid Texas license in Physical Therapy
• In good standing with all regulatory agencies and licensing boards
