Job summary

Wathenahrc is seeking a qualified Physical Therapist to develop and oversee individualized therapy programs in a skilled nursing facility setting, focusing on patient assessment, treatment planning, and progress toward functional goals.

You will collaborate with health professionals and staff, document evaluations and plans, supervise support personnel, and maintain compliance with safety and regulatory standards while delivering compassionate care.

Qualifications

• Graduate degree in Physical Therapy from an accredited program.
• Current license to practice Physical Therapy in the state.
• Knowledge of SNF patient care and regulatory standards.

Responsibilities

• Provide direct patient care, evaluate functional needs and outcomes.
• Design and implement a plan of care based on assessments with family involvement.
• Conduct therapy within standard of care.
• Supervise support staff to achieve patient goals.
• Communicate effectively with clinicians and staff to optimize outcomes.

Job Summary:

The Physical Therapist is responsible for the development of an individualized therapy program consisting of physical assessment, treatment, planning, implementation, education, and communication to maximize a patient\'s progress toward achieving functional goals. The Physical Therapist will also maintain a positive working relationship with other health professionals and ancillary staff.

Duties/Responsibilities:

• Provides direct patient care, evaluates functional needs and outcomes, and consults with other specialists. Reviews patients’ functional needs and adjusts care plans to ensure optimal patient outcomes.

• Designs and implements a plan of care for patients based on a thorough assessment of potential development and incorporates participation by significant others whenever it seems appropriate.

• Conducts therapy practice within defined standards of care.

• Directs the assistance of support staff to achieve patients’ functional goals.

• Communicates effectively with other professional and support staff to achieve positive patient outcomes.

• Practices facility and departmental policies and procedures, objectives, quality improvement program, safety, environmental, and infection control standards.

• Documents evaluations, treatment goals, and plans. Regularly updates documentation and maintains prescriptions and signatures.

• Enhances professional growth and development of self and others through participation in educational programs, in-service meetings, and workshops. Keeps up to date on professional literature.

Education and Experience:

• Graduate of an accredited Bachelors, Masters, or Doctorate in Physical Therapy.

• Proficiency with all applicable state, federal, and professional standards

• Current license from the state Board of Physical Therapy Examiners.

Physical Requirements:

• Duties require extensive standing and walking.

• Requires lifting, positioning, pushing, and/or transferring patients.

• Duties require frequent reaching, stooping, bending, kneeling, and crouching.
