Brooks Rehabilitation in Jacksonville, FL is seeking a skilled Physical Therapist to join the Outpatient team on a part-time basis (20 hours/week). You will evaluate patients, develop individualized treatment plans, and deliver evidence-based therapy in a collaborative, patient-centered environment.

Qualifications

• Current PT license in the state of Florida.
• Current hands-on CPR/BLS Certification.
• Knowledge of federal and state professional requirements.

Responsibilities

• Complete the new patient evaluation, establish customized treatment plans and patient goals, and participate in the implementation of patient care programs.
• Deliver high quality patient care for multiple rehabilitation diagnoses.
• Provide a positive patient experience through patient engagement and progressive treatment.
• Effectively communicate both verbally and written.
• Promote evidence-based patient care.
