Well Care Health in North Carolina seeks a licensed Physical Therapist to assess, plan, implement, and evaluate patient care in home health. You will communicate findings, document per policy, and support program operations while prioritizing patient outcomes.

Qualifications

Responsibilities

  • Assess, interpret, plan, implement, and evaluate patient care by age and diagnosis.
  • Admit patients to the agency when PT is a primary need.
  • Communicate and document in line with agency guidelines.
  • Support program development and discipline-specific operations.
  • Organize work and manage time to improve agency performance.
  • Maintain schedule and contribute to service efficiency.
  • Demonstrate agency values daily in interactions.
  • Foster positive relations with patients, families, and staff.
  • Promote customer satisfaction and quality care.
  • Uphold mission, vision, and values in daily practice.
  • Meet physical requirements and drive to patient homes as needed.
  • Work 40 hours per week, Monday–Friday with weekend/holiday variability.

PRIMARY JOB DUTIES1. Assesses, interprets, plans, implements, and evaluates patients with appropriate level of skill given the patient’s age and diagnosis. Admits patients to the agency when P.T. is a primary skilled need.2. Assists with program development and discipline specific operations efficiently and productively.3. Appropriately communicates and documents information in accordance to agency policies and procedures and discipline-specific guidelines.4. Contributes to program effectiveness.5. Organizes and performs work effectively and efficiently.6. Maintains and adjusts schedule to enhance agency performance.7. Demonstrates a daily commitment to the values of the agency.8. Demonstrates positive interpersonal relations in dealing with all members of the agency.9. Maintains and promotes customer satisfaction.10. Effectively demonstrates the mission, vision, and values of the Agency on a daily basis.JOB SPECIFICATIONS1. Education: Graduate of Physical Therapy program from an accredited university.2. Licensure / Certifications: Licensure in the state in which providing therapy (NC/SC); CPR certification.3. Experience: Minimum 1 year.4. Essential Technical / Motor Skills: Knowledge of home health and physical therapy procedures, ability to manipulate patient care and evaluation equipment, including modalities and assistive devices; ability to properly transfer and guard patients; ability to apply other manual therapy techniques.5. Interpersonal Skills: Ability to deal with the public, co-workers, patients, families, and physicians in a professional and positive manner.6. Essential Physical Requirements: Requires significant locomotion, frequent lifting greater than 50lbs and transferring patients greater than 100lbs. Also repeated reaching in front of the body, bending and stooping, pushing and pulling. Occasionally requires reaching overhead, stair climbing, sitting and fine motor manipulation.7. Essential Mental Abilities: Ability to apply physical therapy clinical principles and use professional reasoning to develop appropriate treatment programs. Ability to prioritize duties, learn new skills and techniques in patient care. Ability to learn and use computer technology and supportive services.8. Essential Sensory Requirements: Ability to see and communicate clearly in order to perform basic clinical techniques.9. Exposure to Hazards: Noise, congestion, communicable disease, exposure to blood borne pathogens, patient assault, and injury resulting from improper body mechanics, uncooperative patients, families, and community hazards.10. Hours of Work: 40 hour work weeks, variable Monday-Friday, weekends and holidays as needed.11. Population Served: Adolescents, adults, geriatrics, and pediatrics.12. Must have a valid North Carolina driver’s license and an operational vehicle.
