Job summary

Boost Home Healthcare is seeking a licensed physical therapist to provide physician-prescribed home-based therapy, aiming to improve or minimize residual disabilities and return patients to their optimum level within their capability.

You will perform initial assessments, develop and modify plans of care, and reassess every 30 days. You may supervise staff and collaborate with the care team to ensure quality, compassionate home health visits.

Qualifications

• Bachelor’s/Master’s/Doctorate in Physical Therapy from an approved program.
• Active physical therapy license in the practicing state.
• One year of home health experience preferred; Medicare OASIS knowledge a plus.

Responsibilities

• Provide physician-prescribed physical therapy.
• Improve or minimize residual physical disabilities for patients.
• Return individuals to their optimum level within their capability.
• Participate in patient care planning with the team.
• Direct and supervise personnel as required.
• Initial assessment, plan of care, maintenance program development, and 30‑day reassessments.

What you'll be doing:

• Provide physician prescribed physical therapy.
• Improve or minimize residual physical disabilities of the patient.
• Return the individual to optimum and productive level within the patient’s capability.
• Participate with all other home care personnel in patient care planning.
• Direct and supervise personnel as required.
• Responsible for initial assessment, plan of care, maintenance program development and modifications and reassessments every 30 days.

What we're looking for:

• A passion to serve and help others live their best lives possible.
• Graduate with a Bachelor's, Master's, or Doctorate in physical therapy from a physical therapist curriculum approved by the Commission on Accreditation in Physical Therapy Education, American Physical Therapy Association, or the Committee on Allied Health Education and Accreditation of the American Medical Association, or the Council of Medical Education of the American Medical Association and American Physical Therapy Association.
• Currently licensed in the state in which practicing
• One (1) year experience preferred in a home health setting
• Knowledgeable and experience with Medicare OASIS documentation

• 68-76$ per visit
• Monday through Friday
• Day Shift
• Weekends as needed
