Job Details: Job Location: Fillmore County Hospital - Geneva, NE 68361, Position Type: PRN, POSITION PURPOSE:
Administers therapeutic treatments utilizing physical agents, biomechanical and neurophysiological principles and assistive devices in relieving pain, restoring maximum functions and preventing disability following disease or injury.

QUALIFICATIONS

SECURITY/ACCESS
  • Will have access to the facility 24 hours a day.

Essential Job Duties and Responsibilities
  1. Evaluates, plans and administers treatment to patients in accordance with physician orders, and accepted standards of professional practice.
    • Establishes a system to provide timely and accurate rehabilitation assessment.
    • Evaluates treatment efficacy and patient profess and makes any necessary adjustments in the care plan to optimize patient care.
    • Educates family and/or support system to functional needs and equipment/resources to meet those needs.
    • Utilizes outcome measurements to objectively assess effectiveness of treatment interventions.
    • Incorporates plans for discharge in treatment plan in collaboration with patient and support system.
  2. Maintains complete and accurate records pertinent to patient treatment and progress to assure proper documentation and to be in regulatory compliance.
    • Documents assessment, treatment goals, and plan of care according to regulatory guidelines.
    • Maintains knowledge of regulations and fiscal changes to optimize organizational resources.
    • Assures the safety and efficacy of equipment used to provide rehab services.
  3. Maintains effective communication with Manager Physical Therapy and other health care providers regarding patient care decisions.
    • Evaluates, records and reports patient progress for review by other members of the plan-of-care team of the hospital.
    • Attends and participates in family conferences, patient care conferences, medical rounds and as required by areas of services to establish goals, formulate discharge summaries and follow-up plans, including home programs.
  4. Follows HIPAA regulations and ensures that the confidentiality of patients medical, personal, financial records, employee records, and organizational records is maintained.
  5. Participates in FCH committees, quality assurance, performance and quality improvement initiatives, and activities which support the facility and department operations.
  6. Complies with the state and federal regulatory requirements related to the performance of FCH operations and requirements of the FCH Compliance Program. Including but not limited to:
    • The Compliance Policy.
    • Completes all on-line training annually and as assigned.
    • All department and organization-wide policies.
    • Considers the impact of regulations such as Critical Access Hospital requirements, along with Federal and State of Nebraska regulations and discusses concerns appropriately to ensure compliance.
  7. Presents self in a professional manner and enhances professional growth and development through participation in education programs, current literature, in-service meetings and workshops.
    • Seeks opportunities for continued growth and performance improvement.
    • Always maintains a professional appearance and manner.
    • Dependable and completes work in a timely manner.
