Job summary

Brookdale Healthcare Services is seeking a Per Diem Physical Therapist with Home Health experience to provide therapy services in the Friendswood, Texas area. Join a dynamic team focused on health, wellness and prevention for our patients.

The role requires a Bachelor of Science in Physical Therapy, TX license, and at least one year of Medicare Home Health experience. Flexibility to work weekends/holidays is preferred as patient needs dictate.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• NBCOT/NCBPT board exam (National Certification Examination) completed.
• Active Physical Therapy license in Texas.
• At least 1 year of PT experience in Medicare Home Health.

Responsibilities

• Provide and direct physical therapy services to patients, including assessment, treatment, program planning and implementation.
• Coordinate treatment with patients, families and other team members.
• Develop home or community programs to support recovery in the patient’s environment.
• Work with physician orders and Brookdale policies and state regulations.

Responsibilities

• Provide and direct physical therapy services to patients, including assessment, treatment, program planning and implementation
• Work in accordance with physicians' orders and adhere to applicable principles and practices of physical therapy, Brookdale policies/procedures, and state regulations
• Conduct patients' screenings regularly to determine need for intervention and treatment
• Coordinate treatment plan with the patient, family and other team members
• Develop appropriate home or community programming to support the patient's continued recovery in his or her own environment

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program
• Completion of the National Certification Examination for Registered Physical Therapist
• A Physical Therapy license within the state
• One year of PT experience or the equivalent; must have experience working in a Medicare Home Health environment
• Ability to work varying hours, including weekends and holidays
