Job summary

Enhance Rehabilitation is seeking qualified Physical Therapists to join our dynamic team in SNF settings. You will evaluate and treat patients following an individualized plan of care, delivering high-quality therapy and compassionate service to promote recovery and independence.

You will collaborate with facility staff, supervise PTAs, re-evaluate progress, adjust plans of care, and complete timely documentation including evaluations, re-evaluations, discharges and daily progress notes to

Qualifications

• Licensed Physical Therapist with current credentials.
• Demonstrated ability to develop and implement plans of care.
• Experience supervising Physical Therapy Assistants preferred.

Responsibilities

• Provide evaluation and treatment for SNF patients following individualized plans of care.
• Collaborate with facility staff to ensure high-quality patient care.
• Evaluate new patients, set functional goals, and document progress.
• Educate patients on injuries, progress, and expected outcomes.
• Re-evaluate patient progress and revise plans of care accordingly.
• Complete timely documentation including evaluations, re-evaluations, discharges, and daily progress notes.

• Collaborate with facility staff to ensure excellent care for you patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Supervising Physical Therapist Assistants
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
